Job Description
We're seeking a dedicated Clinical Pharmacist to join our dynamic team of clinicians, analysts, and support staff. In this role, you'll leverage our innovative software platform to enhance patient care and optimize medication therapy. Your expertise will be instrumental in providing high-quality medication consultations, collaborating on software improvements, and guiding our Clinical Care support team.
Key Responsibilities:
Patient Care: Evaluate and document patient data using our software platform and other tools. Prepare and present comprehensive reports on patient care and team progress.
Medication Review: Conduct thorough medication reviews and adherence counseling sessions. Review and provide feedback on medication reviews performed by other Virtual Pharmacists.
Clinical Shifts: Staff various clinical shifts and engage with new technologies and automation.
Communication: Respond to inquiries from patients, providers, and team members. Follow up on patient issues and concerns, ensuring timely resolution.
Process Improvement: Participate in meetings to refine workflows and enhance our software platform. Develop strategies to improve medication utilization and meet quality measures.
Team Collaboration: Guide and support other Clinical Care team members. Orient new Virtual Pharmacists and assist in developing clinical algorithms.
Regulatory Compliance: Adhere to all relevant policies, rules, and regulations.
What We're Looking For:
Qualifications: Doctor of Pharmacy (PharmD) from an accredited institution with an active California State License. Completion of a PGY-1 residency or equivalent experience.
Skills and Experience:
Proficient in reviewing and assessing prescriptions with consideration of various patient factors.
Strong ability to engage with patients, promoting adherence with empathy and understanding.
Excellent organizational skills and the ability to prioritize effectively.
Knowledge of chronic diseases such as diabetes, heart failure, COPD, and more.
Ability to work independently and make informed decisions.
Experience working with diverse patient populations and understanding their unique needs.
Familiarity with local, state, and federal regulations, as well as prescription and non-prescription drugs.
Willingness to obtain licensure in additional states if needed.
Strong written and oral communication skills and proficiency in Microsoft 365 and Google Workspace.
Ability to work a full-time schedule of 40 hours per week.
Preferred Qualifications:
Experience with electronic medical records.
Board certification in a specialty area.
Experience in a busy healthcare setting.
Bilingual in English and Spanish.
